(TELLER COUNTY, Colo.) — A review by numerous safety administrations has concluded and determined the death of tour guide Patrick Weier at the Mollie Kathleen mine in October of 2024 was accidental due to operator error. According to a press release from the Teller County Sheriff's Office (TCSO), the Colorado Department of Labor and Employment’s Division of Reclamation, Mining, and Safety, the Division of Oil and Public Safety, and the Mine Training and Safety Program completed its review of the Oct. 10, 2024 incident which left 46-year-old tour guide Patrick Weier dead and trapped 12 others underground for several hours. PREVIOUS COVERAGE: Mollie Kathleen Mine undergoes compliance inspection The safety inspection was prompted by a cease-and-desist order placed on the tourist mine’s operation following the incident. TCSO said the inspection looked at the mine’s ground control, inspection and training records, ventilation, fire prevention and control, communication systems, personnel hoisting mechanisms and procedures, and other safety and health requirements. "State mining inspectors determined that the Mollie Kathleen tourist mine met the provisions of the Regulations for the Mine Safety and Training Program for Tourist Mines and no imminent or substantial danger to the public or employees exists," the press release states. In addition, an investigation by TCSO determined that the death of Weier was accidental and caused by operator error, and not caused by any mine practices or equipment malfunctions. "The case has been closed as an accidental death," TCSO said. "The cease-and-desist order of October 2024 has been lifted, and the Mollie Kathleen Mine will now be able to resume operations for the tourist season of 2025."
