The Oval Office was staged for a victory lap on Friday, Sept. 18. Six officials stood shoulder to shoulder behind the Resolute Desk, the cameras were live, and the White House had a number it wanted repeated all weekend. Everybody in the room hit their mark. The man sitting at the desk did not. For long stretches of the event, Donald Trump‘s eyes kept sliding shut while the people he had invited took turns thanking him. By the weekend, though, the heavy eyelids were only half of what people were replaying. Trump declared that all 50 states, Washington, D.C., and Puerto Rico had signed on to the Medicaid “GENEROUS” model, which ties the price Medicaid pays for certain brand-name drugs to the lower prices paid by other wealthy countries. The Council of Economic Advisers put the projected savings at roughly $64 billion over a decade. Kennedy Jr., CMS Administrator Dr. Mehmet Oz, Arkansas Gov. Sarah Huckabee Sanders, Mississippi Gov. Tate Reeves, South Dakota Gov. Larry Rhoden and CMS innovation director Abe Sutton. It landed as affordability dominates voter attention ahead of November, months after congressional Republicans let Affordable Care Act subsidies lapse and premiums climbed. But the room kept pulling focus away from the policy. One social media user posted footage recorded while Oz was speaking, writing that it is not normal for a president to be unable to stay conscious during televised Oval Office events. In a second clip, shared on Threads, Sanders delivers her remarks inches from Trump’s shoulder and his eyes close completely. Nobody behind him reacts though. Then, in a separate stretch of the same footage, the 80-year-old appears to wake up and immediately reach for something. With both hands folded on the desk in front of him, Trump begins slowly working the fingers of one hand over the back of the other. He’s seemingly pressing, smoothing, circling the exact patch of skin that has been the subject of two years of speculation.  Then he quickly glances down at his hands, seemingly checking whether the makeup concealing the bruise had rubbed off onto his other hand. He keeps at it while Oz talks, then flattens his left hand over his right and leaves it there. That patch is not new. Zoomed-in photos going back to August 2025 have repeatedly shown the back of Trump’s right hand covered in a layer of light foundation, and bruising visible underneath it when the makeup wears thin. The White House has attributed the discoloration to frequent handshaking combined with the daily aspirin Trump takes for cardiovascular prevention. On social media, the clip did what these clips always do. “He’s not listening to you Sarah,” one commenter wrote beneath the video on Threads.  A third kept it short: “I hope the back of his hand is oozing pus and hurts like hell.” Another pointed at the irony: the governor was “looking right at trump sleeping and acting like it’s not happening,” they wrote, adding, “And that woman had the nerve to call Biden, Sleepy.”  Under the hand footage, the guesses got more specific. “Must be better camouflage for his IV,” one person speculated. Another reached for a comparison recently incapacitated 84-year-old Republican Sen. Mitch McConnell that has followed Trump for months: “Probably thinking about Mitch’s hands and wondering if his will be that bad.” The White House published full video of the health care announcement, and its accompanying materials addressed Medicaid pricing only. Neither Trump’s alertness nor his hands were mentioned. This is well-trodden ground by now. The Daily Beast counted at least 20 public occasions on which Trump has appeared to nod off, including an Aug. 31 drug-pricing event where his eyes stayed shut for more than 20 seconds.  Newsweek‘s running timeline logs a Memorial Day ceremony at Arlington, an Oval Office meeting with travel executives, and a November 2025 announcement during which he appeared to struggle for nearly 20 minutes.  After that one, a White House spokesperson insisted that Trump was not sleeping and had spoken throughout. The most recent published medical disclosure predates Friday by months. On May 29, physician Sean Barbabella reported after an exam at Walter Reed that Trump’s leg swelling had improved, scored him 30 out of 30 on a cognitive screening, and declared him in excellent health and fully fit for duty. This comes after Trump’s swollen, discolored hands had previously raised alarms in May. Two months before that, he sat stone-faced through a glowing tribute until the man beside him noticed something was off.
